Book review Dr. Differences in health within and across population groups exist as a result of multiple, interconnected factors. These factors range from the individual risk of disease all the way to the social and economic policies that shape our context and the characteristics of the communities we work and live in. The multilevel nature of the determinants of population health is a relatively easy concept to grasp, but it is much more challenging to understand how these determinants are precisely connected at multiple levels. Complex systems methodologies such as agent-based modeling, system dynamics, and discrete event simulation have been around for decades, but their application to understanding the causes and consequences of health disparities is much more recent.

This report is very welcome and timely. The contributions of modern science and technology to the quality of modern healthcare are everywhere visible. They include non-invasive imaging, minimalist surgery and new anaesthetics, stents and statins, genetic testing and new techniques and treatments for cancers. There is much more to come as we head into the era of personalised medicine in which diagnostics and treatments are moulded with much greater accuracy around the individual. Less obvious, but equally as important, has been the contribution of the social sciences, as evidenced by the examples in this report. Healthcare is enormously expensive.

THE HEALTH OF PEOPLE

This report by the Agency for Healthcare Research and Quality AHRQ examines behavioral health epidemiology, or the study of the effects of behavioral and social factors on various aspects of health and well-being, such as longevity, disability and illness, and population-level quality of life. The report is organized into five sections to explore social epidemiological perspectives, policy change, biological influences, economic investment, and development of research tools. It recommends a wide range of changes to improve behavioral health research methods. Kaplan R et al. It first examines current prevalence and trends in the status of violence and bullying, including factors, types of violence, changes over time, consequences and regional investigations.

Nationally, there is increasing recognition to shift away from a paradigm of reducing health disparities in vulnerable populations to one of advancing population health equity for all. In clinical settings, a comprehensive assessment of SDH can potentially yield more contextualized information on targeted strategies for patient populations living with complex health and social conditions. Despite broad consensus that social and behavioral factors impact clinical outcomes, surprisingly few studies have identified direct associations between patient-level SDH and negative health outcomes. To address this critical lack of evidence, Zulman and colleagues 3 undertook innovative research to identify discrete SDH factors, including behavioral factors at the individual level, that could improve the modeling algorithm used by the US Veterans Administration VA electronic health record EHR to estimate risk of hospitalization among high-risk patients across the VA health system. As one of the largest integrated health systems nationwide, the VA is uniquely positioned to test practice-based innovation. The VA benefits from a systemwide EHR that has been used successfully for patient management and analytics for more than 20 years and serves a high proportion of complex patients. By committing substantial financial and human resources to the development and delivery of data-driven best practices, the VA has tested, implemented, and evaluated patient-centered medical home models for high-risk and vulnerable patients, including veterans who are homeless or have a mental illness, that have provided the blueprint for successful interventions in health systems nationwide.
