The foot pressure ratio and static balance scores during standing were measured using a SpaceBalance 3D system. Under the 3 conditions tested, the static balance score was higher for the revised high-heeled shoes than for the general high-heeled shoes, but this difference was not statistically significant. High-heeled shoes HHSs are defined as shoes in which the heel is higher than the forepart. HHSs often also include a narrow toe box, rigid heel cap, and curved plantar region, all of which interfere with natural foot motion. HHSs have been worn for several centuries 1 , and despite numerous cautions against their use, they remain extremely popular. The use of HHSs cause abnormal postural alignment, particularly in the lower limbs and spine 3. Thus, increased forward head posture, lumbar hyperlordosis, pelvic anteversion, and valgus knees.
Some doctors and therapists believe that wearing high-heeled shoes causes increased lumbar lordosis and that this may be a cause of low back pain. The purpose of this study was to evaluate whether high-heeled shoes increase lumbar lordosis and to do so with more reliable methods and a larger sample size than used in previous studies. Fifty participants from a chiropractic university were included in a test group 32 female and 18 male and 9 in a control group 3 female and 6 male. A Spinal Mouse was used to measure lumbar lordosis in test participants barefoot and then again with 3- or 4-in high-heeled shoes after a minute adaptation period of walking and sitting and standing while wearing the shoes. Reliability of the testing conditions was evaluated with 9 barefoot control participants before and after an identical adaptation period, and intra- and interexaminer reliability of Spinal Mouse measurements was tested by use of a wooden model built to mimic the proportions of a human spine. Both groups showed non-significant decreases in lordosis between the first and second scans high heels: Consistent with most previous studies, high-heeled shoes did not affect lumbar lordosis in most people while standing.
Some women complain of low back pain that they believe is due to wearing high-heeled shoes, and some clinicians seem to think the reason is that high-heeled shoes cause increased lumbar lordosis. This article examines Internet information aimed at the general public and presents a literature review of available research in this area. The keywords high heels and high-heeled shoes , combined with the words lumbar , lordosis , and pelvic tilt , were used in an Internet search of Ask. There are many Internet sites that support the belief that high-heeled shoes cause increased lordosis. However, published research for this topic mostly does not support this belief; but some mixed results, small subject groups, and questionable methods have left the issue unclear. It appears that some health care providers are offering advice about the effect of high-heeled shoes on lumbar lordosis that conflicts with most published research. However, the prevalence of such advice is unknown; and the published research is equivocal.
Measurement of lumbar lordosis in static standing posture with and without high-heeled shoes
We examined 16 healthy women, 21 to 23 years of age, who stood unilaterally on the right foot with the heel elevated 0, 5, 10, 15, 20, 25, 30, and 40 mm. We measured height of the medial longitudinal arch, rearfoot pronation calf-to-calcaneus angle , displacement of center of pressure under the foot during ten seconds, and mean position of the center of pressure during those ten seconds. Elevating the heel raised the medial longitudinal arch, reduced rearfoot pronation, increased length of displacement of the center of pressure, and brought the mean center of pressure forward and medially. In feet that initially had strong rearfoot pronation, presence of an initially high medial longitudinal arch was associated with a greater curtailment of rearfoot pronation when the heel was elevated than if the arch was not initially high.
